• Please complete this online form to provide us with your chapter name, the organization who will benefit and a description of your community service project.

• All funding requests must be designated for a community service project that will benefit a named organization.

• Chapter may submit more than one request for matching funds - each request must be submitted separately.

• Approved projects will receive IMC USA up to $500 in matching funds for a potential total project cost of $1000. ($500 chapter funding - $500 IMC USA funding.

• Projects must be completed by September 30, 2015.

• Requests for matching funds will be reviewed within 3 business days of submission.

Examples -
(1) Chapter X will provide 5 or more volunteers to perform a one day project planning exercise for XYZ non-profit organization. The community service event will coincide with our normal chapter meeting thereby increasing member exposure and participation. Expected outcome is that XYZ non-profit will have a high level project plan for addressing its ABC challenge.

(2) Chapter X will provide 6 or more volunteers for a full day on a Habitat for Humanity build project. Expected outcome is that the chapter will donate $1000 (one-half from national) for building materials.
